ZERO TOLERANCE

This practice has ZERO TOLERANCE towards VERBAL and PHYSICAL aggression. Any patient who is seen to be aggressive will be asked to leave the premises and will be permanently removed from the patient list.

PRESCRIBING OF PAIN & SLEEPING MEDICATIONS

This practice will not prescribe pain or sleeping medication at your first appointment, via a phone requests, without a proper assessment or over the long-term.  This exception is for terminal cancer

FAILURE TO ATTEND POLICY

Practice policy requires 3 hours notification of a cancellation or a Failure to Attend Fee of $30 will be incurred and payment will be required before another appointment can be booked.

Daintree is a mixed billing clinic with both bulk billing and private billing consultations.

Bulk Billed appointments are provided for ALL Concession card holders (Pension concession and Health care cards), DVA patients and Children 16 Years and Under.

Private Billing is applied for all other standard consultations of 15 minutes or long consultations of 30 minutes and will incur a $30 out-of-pocket fee. The full amount is payable on the day (see fees table)  and our friendly reception staff will assist you in claiming back your Medicare rebate.

Workcover, TAC, medical reports, insurance and pre-employment checkups are not covered by Medicare and will incur a private fee. Fees will advised at time of booking.

All out of pocket fees will advised at time of booking.

Appointments with Allied Health providers are not always covered by Medicare. Please check with Administration staff for further information.

The practice runs on booked appointments. Please be aware that appointment timings vary. Please be conscious of the doctor’s and other patient’s time and make appropriately timed appointments. If you wish to discuss more than 1-2 issues with your doctor, please book a longer appointment. The GP’s make a conscious effort to run on time but in the event of emergencies, the reception staff will endeavour to inform you the approximate waiting time frame. Please bear in mind this is an approximate estimation of time only.

If you wish to have Pap Smears or Immunisations, please be aware this requires to be notified in advance so you can be booked in with the relevant health professional in the clinic with appropriate consulting time.

We are pleased to hear from you at least 3 hours before your appointment, but not 10 minutes after you have missed your appointment! The clinician’s time is wasted every week because of simple failures to attend. Please cancel any unneeded consultations as soon as you know you must – others are desperate for the doctor’s time.

For the benefit of all, it is practice policy that, if you fail to attend an appointment, and neglect to call practice advising of cancellation or rescheduling, there will be a fee of $30 to $90 per missed appointment depending on the clinician you were booked to see. If you miss 3 appointments in the space of a year, if you fail to attend for longer appointments amounting to 30 minutes in a year, or if you fail to attend any appointment soon after joining us, we will contact you to advise you of this and how this breaks down the special relationship you need to have with your doctor.

Practice policy is also that further failures to attend are monitored, and will mostly result in patients being invited to seek GP services elsewhere. We stand by our efforts to deal with this major problem in the interests of the vast majority of our patients.

We do not give results over the phone due to patient confidentiality.
If you wish to discuss results with your GP, please feel free to book an
appointment. If your results have been marked for discussion, you will
be notified accordingly by the staff and arrangements made to have an
appointment. All urgent results marked for follow up will be dealt with
accordingly.

Appointments are required for script renewals and are available as Private Billing. This will incur a reduced out-of-pocket fee of $10. The full amount is payable on the day and our friendly reception staff will assist you in claiming back your full Medicare rebate immediately.

Daintree Medical Centre runs on booked appointments.  A standard appointment is booked for 15 minutes.

Appointment timings vary and often involve a GP and Practice Nurse working together to provide the best patient care.

Please be conscious of the doctor and patients’ time and make a long appointment (30 minutes) if you wish to discuss more than 1-2 issues with your doctor or request a script renewal-only appointment with your usual GP.

Our GPs make an effort to run on time but in the event of unforeseen circumstances and patient emergencies the reception staff will endeavour to inform you of the approximate waiting time. Please bear in mind this is an approximate estimation of time only.

Bookings can be made by phoning 02 6059 2700 from 8.00am MON-FRI and 9.00am SAT.

Please advise if you require a non-standard appointment so the appointment is booked with the relevant health professional in the clinic with appropriate consulting time.  Fees and policies will be advised at time of booking.

To avoid inconvenience and disappointment only book standard appointments (15 mins) via online booking on website or APP.

Long and more detailed consultations will need to be made by phone or in person for:

Procedures, Skin Checks, WorkCover, TAC, wound dressings, injections, immunisations, cervical screening, forms, medical checks, removal of stitches, implanon insertion and removal and ear syringing.

All patients require a VALID Medicare card number otherwise a non-rebatable consultation fee will be charged to those with no Medicare card or with an expired card.

Appointments can only be made with a GP and only one appointment per day.

Only one patient can see a GP per appointment.

When you register with Daintree Medical Centre for GP Services your registration is with the practice and not with a specific doctor.

You may consult any doctor at this clinic or change doctors within the clinic at any time – all doctors have access to the records and notes made by the previous doctor.

When booking an appointment we will endeavour to comply with any reasonable request to give you an appointment with your preferred doctor however on occasions this cannot happen due to availability.  The next available appointment with your preferred GP will be offered and you will be added to the GP cancellation list.  If you require an earlier appointment you will be offered an available doctor in the clinic who can guide you towards the best treatment available.

Please feel free to then make a follow up with the doctor of your choice at their next available appointment.

We are pleased to hear from you at least 3 hours before your appointment.  We understand that at times an appointment may be overlooked or there may be extenuating circumstances that prevented attendance.

Please cancel any consultations that are no longer required as missed appointments have an impact on the availability of appointments for other patients requiring treatment on that day.

Not all services are covered by Medicare.  Please advise if booking for WorkCover, TAC, medical reports, insurance and pre-employment checkups as private fees apply.  Fees will be advised at time of booking.

We do not give results over the phone.

All results are checked by your GP or another GP in the clinic.

If your results have been marked for follow up by your GP you will be contacted to book an appointment.  However if you have concerns and wish to discuss results with your GP please feel free to book an appointment.

The GP may request a non-urgent appointment within 2 or 6 weeks, or an appointment within 24-48 hours.

Our staff can book appointments as indicated by the GP for follow up, however they cannot see the test results.

3 attempts will be made to contact you for follow up of results.  If you are not able to be contacted directly a follow up letter will be sent requesting you make contact with the clinic to make an appointment or advise us that you are attending elsewhere or under specialist care.

Correct patient identification is vital for patient safety, confidentiality and data quality. Patients are asked identification at each encounter with our practice team.  Please use this opportunity to update your details in particular address, contact phone numbers, next of kin and emergency contact.

Your contact details are used for result recalls, SMS appointment reminders (consent) and to contact you if required regarding your appointment.

Please tell us if you have a new Medicare card or change to your concession status.

Please let your GP know of any changes to your health or medication, including allergies.

In extraneous circumstances you can phone your GP.  The concern will be triaged and a message sent to your GP.  GPs will endeavour to address your concern and communicate with you.

Your email will be transferred to your patient file and your doctor will access your request the same way they would any pathology results or specialist letters we receive for you.

Please note that email is not a secure form of communication and should also not be used for matters of urgency

Home visits are undertaken for patients in a residential aged care facility and can be arranged for regular patients of the practice in exceptional circumstances, such as palliative care, acute illness or immobility.  Considerations also include proximity to the practice, safety and alternative access to care.

We much prefer that patients attend the clinic where we have a much greater range of equipment to help us to look after you.

This practice does not disclose your personal health information unless you have consented to this disclosure, or the disclosure is necessary because you are at risk of harm without treatment and you are unable to give consent.

Personal Health Information and your Medical record, may be collected, used and disclosed for the following:

·        Consultations with GPs and Practice Nurses

·        Referrals with relevant information to Specialists and other Allied health professionals

·        Results, recalls and reminders.

·        Medicare billing

·        Infectious disease notification as required by VIC legislation

·        Legal disclosure as required by a court of law (eg subpoena, court order, DHHS)

·        Quality Assurance activities such as accreditation.

·        research purposes (de-identified)

Medical Records can be transferred at a patient’s request under the Health Records Act (2001).  Fees apply as per the schedule.

Daintree Medical Centre welcomes patient feedback and offers suggestion forms at Reception.

Patients are invited to participate in a survey regarding our services using an independent research company and use the results to strive for continuous improvement.
